A door can come off track when it is hit by a vehicle, or when a roller, bracket or cable fails. You may see rollers out of the track, a door sitting crooked, or a gap at one side.

Stop using the door. DASMA's checklist asks whether the rollers stay in the track during operation, and says that if they don't, the inspection should stop and a trained door systems technician should look at the door. Don't run the opener: one US door dealer warns that an off-track door can fall without warning, and that running the opener can snap a cable or make the misalignment worse.

Keep people and vehicles clear, and don't try to hammer the track back or lift the door into place. If a vehicle is trapped, tell the shop first. Ask whether panels, rollers or cables were damaged too, and get the price for that before it's done.
